The Accessible Information Standard aims to ensure that patients (or their carers) who have a disability or sensory loss receive information they can access and understand, for example in large print, braille or via email.
As part of the Accessible Information Standard we must:
If you feel this applies to you, or someone you care for please let us know by telephoning us on 0300 130 3333.
Do you look after someone? Are you receiving everything you’re entitled to? When you’ve cared for an individual for a long time, are temporarily helping them (for example, while they recuperate from an operation), or have just become a carer, take the time to review your options and find out what support is available to you.
A carer is anyone who cares, unpaid, for a friend or family member who due to illness, disability, a mental health problem or an addiction, cannot cope without their support. If you are a carer, please let our team know.
Safeguarding adults and children is important and there may be times when you are concerned about the welfare of a child, adult or both. If you need advice, guidance or support please refer to the Wandsworth CCG website for links to services.
We’re always trying to make our healthcare services even better, and our patients are the best people to help us to do this. Our regular Patient Participation Groups (PPGs) are a great way to make a difference to healthcare in your local community, and we’d love you to join a group.
PPGs help us in all sorts of ways, from ensuring that the information we produce is easy to understand, through to coming up with new ideas for services, or how we deliver them. Simply contact us at email@example.com to find out more and get involved, or fill out the form attached to the Patient Participation Group leaflet and return it to our reception at The Junction.
