Join the practice

We always welcome new patients to register with The Junction Health Centre. To be eligible to join the practice you must live within our SW11 catchment area.

Registrations submitted from outside of the SW11 area will not be processed.

How do I join?

Adults

1. For adults 16 and over, fill out an online adult registration form and submit. If you are unable to register online please collect a new patient form from our main reception.
2. A member of the administration team will process your registration within 3 working days. They may need to contact you for more information so please ensure your contact details are correctly entered.
3. Once the first part of your registration is processed you will receive an SMS or call asking you to attend a new patient health check with our healthcare assistant – this is to ensure we have as much information about your health as possible. Please bring a proof of ID and proof of address to this appointment.

If you need to see a clinician (GP/nurse) you must book a separate appointment.

Children

Children under 16 cannot be registered via the online process and must complete the child registration form and bring to the practice for processing – a parent or guardian must be registered with us too. Where possible please provide the child’s up to date immunisation history.

Once you have registered with our practice, you will have the opportunity to register for online services which gives you easy access to your online patient data all in one place. It couldn’t be simpler.
